AGREEMENT
I understand that if at any time you cannot or do not wish to keep this cat, that I MUST contact Forgotten Felines.
I understand that kittens MUST BE SPAYED/NEUTERED AT 5-6 MONTHS OF AGE
or will be reclaimed by Forgotten Felines.
I understand that Forgotten Felines has the right to reclaim the unspayed female and her kittens if she were allowed to have a litter.
I understand that this cat must be cared for properly and not abused in any way.
I understand that this cat must be provided with necessary veterinary care.
I understand that if the terms of the adoption contract I am signing upon adopting a cat from Forgotten Felines are not followed, Forgotten Felines may reclaim this cat and seek restitution for all expenses incurred. I certify that the above information is accurate and complete.
I authorize the release of veterinary information related to my current and past pets.
I authorize my landlord to verify my ability to house a pet on his property.
Forgotten Felines reserves the right to deny any application.

Thank you for considering adopting a cat from Forgotten Felines. We are responsible for finding the best possible home for this animal. Consequently, applications may take up to 72 hours to process. Please be patient – your new kitty is well worth the wait!
$100 Kitten Adoption Fee
Adult Adoption Fee: depending on age
This fee includes free spay/neuter (or certificate to one of our participating vets when kitten becomes 5 months of age, first distemper vaccination, first worming, testing for feline leukemia/feline aids and no fleas. If you loose the participating vet list, call us at 914-428-7969 or email forgottenfelines@yahoo.com.
Ask about our "long term" fostering of one of our feline leukemia/feline aids kitties who are healthy but in need of a loving home. Here the adoption fee is waived but a vet reference must be provided. Forgotten Felines will pay their vet bill related to their illness if you bring the kitty to one of our partcipating veterinarians. You provide the food, toys, and most especially, love.
